Your First Visit*
Please arrive about 10 minutes early so that you have enough time to complete any necessary forms. You can fill out a New Patient form at your first appointment, or download, complete, and bring it with you.
*IMPORTANT: Any form submitted by email must be encrypted, to ensure it is secure. By submitting an unencrypted email form, you are consenting to an unsecured email. Alternatively, please print off the form, complete it, and bring it to our office.
To begin, your dentist will review your medical and dental history and complete a comprehensive dental exam. This exam will include a detailed examination of your teeth, bite, jaw and gums, any x-rays that may be required, and an oral cancer screening.
Your dentist will create a long-term dental treatment plan for you based on your examination and history. There will also be time allotted for you to ask any questions you may have about your dental health and treatment options.
